An interesting phenomenon with revolvers was discussed by Tam:
High recoil causing subsequent rounds to disassemble from weak crimps.
Essentially, the recoil acts like a bullet-pulling hammer and causes the bullet to separate from the remaining rounds in the cylinder.
Another revolver malfunction was reported by a caller to last Sunday’s GunTalkTV radio show:
The caller described what may be the result of an oversized or stretched cylinder:
When pointing the revolver up in elevation, a cartridge would slip rear-ward, preventing the cylinder from rotating.
Evidently, even revolvers can have failure to feed issues.
The best advice I can offer is to remain on high alert at all times with ANY firearm and to investigate immediately the cause of any abnormal operation.
To ensure your guns are in top condition and working properly, shoot them often so you can tell that they are performing differently than ‘normal’.
If problems are identified, have them corrected immediately.

Certain cartridges can set back during recoil and bind up the cylinder as well.
I’ve seen rounds disassemble under recoil, that’s why I always do a strong crimp.
